If you do not have a SystmOnline account, you can use this form to request any repeat prescriptions from the Practice. A repeat prescription is a regular medication that is on your repeat list. Short term medications for acute problems, e.g. antibiotics for an infection, cannot be requested as a ‘repeat’ and a triage appointment should be booked. This is so that we can make sure we prescribe the right medication for your current problem.

Please allow 4 working days before collecting your prescription.

In future you may wish to consider registering for our Online Services. The Online Services system remembers which medications you are on and makes requesting repeat prescriptions faster and easier. We are no longer able to take prescription requests by phone or fax.
