Self referrals
There are a number of services locally that you are able to make self referrals to.
- Drugs and alcohol
- Minor eye conditions
- Mental health
- Physiotherapy
- Stop smoking
- Termination of pregnancy
- Social prescribing
GP referrals
To request a GP referral:
- phone us on 01525 715300
- Submit a request using the Accurx online form explaining why you need a referral. You may need to have an appointment with a GP to discuss your request. We will advise you if this is the case.
Provide as much information as you can as this will help us to process your request.
Tracking a referral
To track an existing referral:
- phone us on 01525 715300
- Submit a request using the Accurx online form stating details of your referral such as the specialism you have been referred to, hospital and date of referral.
